New Patients Welcome

We welcome new patients for both Family Care and Psychiatry / Mental Health Services. Our staff will guide you through the intake process and answer any questions you may have before your first visit.

What to Bring to Your First Appointment

Please bring the following:

  • A valid photo ID
  • Your insurance card (if applicable)
  • A list of current medications
  • Relevant medical or mental health records (if available)
